Please be sure to notify our reception with any changes to your contact information such as your address and phone number. It is essential that we keep the correct information on file for follow up calls and letters in the mail.

We are not a walk-in clinic, so patients must have an appointment to see a doctor, nurse, dietician, social worker or another specialist. However, we are always available to talk to you about an urgent problem.
Please bring all your medications with you to your appointment with the doctor.
A limited number of same-day appointments are available. Please limit your consultation to the specific issue that is the reason for the same-day appointment, as same-day appointments are often double-booked to accommodate more patients, and may, therefore, be shorter than usual.
Missed appointments take valuable time away from ill people who are waiting to see their doctor. Please provide advance notice for appointment cancellations. Failure to adhere to the advance notice policy will result in a missed appointment fee of $50.00.
Please be on time for your appointment. If you are running late, please call the office and we will try to accommodate you, but we cannot guarantee that the physician will be able to see you if you are more than 10 minutes late.
The Ontario Health Insurance Plan (OHIP) does not pay for the following uninsured and third-party services.

“No news is good news”.
Due to the overwhelming number of test results received each day, we will usually only notify you if your test result is abnormal. You may phone and inquire about your result if you have a particular concern or book a follow-up appointment with your doctor to discuss your concerns.
If your physician initiates a referral to a specialist, please note, the wait times for a specialist appointment vary widely. Once we have confirmation of your appointment, our office will notify you either by telephone or by letter. We will outline the appointment time, date and location and any specific instructions you are required to follow. Please ensure our office has your current demographic information.
If you have not heard from our office within four weeks you may call the office to inquire on the status of your referral.
It is the patient’s responsibility to organize their medication refills. All prescriptions requested without an appointment are subject to a $20.00 fee. If you are on your last refill and have no appointment booked, you should contact us the latest 2 weeks in advance. Any renewal requests from your pharmacy are subject to a $20.00 admin fee as well as a request over the phone without a booked appointment.
All patients on narcotic medications (including, but not limited to Tylenol with Codeine, Codeine, Morphine [Statex, MS Contin, Kadian, etc.] Fentanyl, Hydromorphone, [Dilaudid etc.], Percocet, Endocet, OxyContin, etc.]) may be asked to sign a treatment contract and may be asked to provide unannounced urine samples. If a patient declines signing the contract, providing a urine sample, or if a urine sample is failed, the physician may stop prescribing the medication without warning.
Since our clinic receives OHIP funding for allied health professionals (counselors, dietitian, nurse practitioner, clinic pharmacist) to be able to deliver comprehensive primary care, our physicians are charged for your visit to any family physician outside our clinic. This includes walk-in clinics, some weight loss clinics (i.e. Bernstein) and some pain clinic (i.e. AIM). If you are planning to use any family physician’s services outside our clinic, please discuss this with our staff in advance. OHIP sends us a list of those patients and the date(s) they used services outside our clinic. This allows us to contact you after we paid for your outside visit.
